Having built a pilot community-based care management program for a California-based Health Plan from scratch, we are able to approach the development of your agency plan with innovation and compliance with regulations.

Successfully conceptualized, developed and submitted Round 1 grant applications for six (6) ADHC/CBAS programs. All 6 applicants were awarded funding with an average $80,000 in grant awards.
